    Street address should be filled out in Line 1 & 2 fields only. City, State and Zip Code MUST BE PROVIDED in order to submit form
    Please indicate specific freight account information for delivery (UPS, FedEx, DHL, etc)
    Please Note:
    1. We will email you with an RMA number.
    2. Please include customer ticket stock for final testing of the repaired printer.
    3. Re-attach any missing doors or covers.
    4. Please write the RMA number on outside of shipping box. Boxes without RMA numbers may not be processed promptly.

    Non-warranty repairs are subject to a $50 estimate charge, which is deducted from the cost of repair.
